Seeking an experienced Care Coordinator to conduct HARP assessments, make referrals to HCBS services and create Plans of Care for individuals with serious mental illness. Assistance with case coverage will be required as needed. The position requires travel throughout the Bronx, Brooklyn, Manhattan and Queens. A bachelor’s degree in a human service related field and 2 years of experience working with this population is required. Spanish speaking a plus.
