Case Managers for Scatter Site Housing Program in Brooklyn Office will work directly with client caseload by providing services to SMI population in visits to client homes and in the community. Will coordinate care and maintain collateral contact with clients’ service providers, provide crisis intervention, and maintain case record around service provision. Must work well independently and take initiative in the execution of job responsibilities. Bachelor’s Degree preferred. Experience working with SMI population required.
