Seeking Case Managers for Scatter Site Housing Program in the Bronx and Brooklyn. Will work directly with client caseload by providing services to severe mentally ill population in visits to client homes and in the community. Will coordinate care and maintain collateral contact with clients’ service providers, provide crisis intervention, and maintain case record around service provision. Must work well independently and take initiative in the execution of job responsibilities. Bachelor’s Degree preferred. Experience working with severe mentally ill population required.
