Seeking experienced case managers to serve as care coordinators for individuals with serious mental illness on an Assisted Outpatient Court Order (AOT) and/or recently discharged from a state psychiatric center. The position involves the provision of intensive case management and accountability for coordinating all aspects of care; significant field work involved.  A bachelor’s degree and 2 years of experience working with this population is required.  Spanish speaking a plus.
